When a player character loses a conflict, part of the consequences of failure can be damage to the character. Any single conflict can only damage a character once. Damage is expressed as a -2 penalty to one kind of conflict. It could be "-2 to running away" or "-2 to fighting" or "-2 to standing up for yourself". Physical damage and damage to self-esteem or confidence is expressed exactly the same.
If a character already has three penalties (i.e. three distinct -2 penalties), then any further damage will take the character out of the scene. It's up to the group whether character death is an option, but if it is, at this stage, failing any deadly conflict will kill the character.
The penalties only apply to a single conflict, and disappear as soon as they are applied. That means that the only way to heal damage is to get into a conflict in which that penalty will apply. GMs who are feeling devious will avoid giving characters penalties to "fighting" (which is easily removed), and will instead give them for "running away" or "seeing more enemies approaching".
